Service Terms

• All included services rendered in-office.
• Appointment required.
• Services must be used in the month (or other period) in which they are earned or will be forfeited.
• Upgrades may be added at the time of service for an additional fee.
• We reserve the right to refuse membership to anyone at our discretion.
• We reserve the right to change the terms of this membership at any time without notice.
• Member discounts are not combinable with other promotional discounts.
• Reward points may not be combinable with member discounts.

Cancellation Terms:

• Cancel in writing
• 60-Day Notice to Cancel
• 6-month minimum commitment
• Fee Charged unless term completed. 
• Membership Fees are Non-Refundable.
• This membership fee recurs monthly unless canceled prior to the end of the term
• A minimum 6-month commitment is required. If membership is canceled prior to the end of the 6-month commitment, the remaining months will be charged to make the 6-month commitment.
• Maximum cancellation fee is $1,089.
